How to Apply
Interested candidates may obtain the application form and prospectus from MGM Institute of Health Sciences, Kamothe, Navi Mumbai on payment of Rs 2000 in cash or Rs 2500 in the form of Demand Draft. The DD should be drawn on any nationalized / scheduled bank in favor of MGM Institute of Health Sciences, payable at Mumbai. One can get the same by paying the amount as per above details from the below mentioned notified centers of MGM Institute of Health Sciences. In addition, one can also download the application form and information brochure from the Institute’s official website: http://www.mgmuhs.com/. NRI (Non Residential Indian) candidates can also apply by paying US $10 (application fee) and US $250 (examination fee) in the form of DD as per above details.
The duly filled in application form, along with a DD of Rs 3000 (examination fee) drawn in favor of MGM Institute of Health Sciences, payable at Mumbai and all requisite documents (list of enclosures given on the website), should be sent to office of the MGM Institute of Health Sciences, Navi Mumbai on or before January 10, 2012. Candidates must write the name and application no. on the reverse side of the DD. In case of downloaded application form, candidates have to attach a separate Demand Draft for application form (Rs 5500) with the completed application form. The application number of the downloaded form will be provided by the Institute to the students after receiving the receipt.
